The Stark County Health Department has access to all birth records that are filed within the state of Ohio. The Stark County Health Department also has death records dating from 1908 to the present year, for persons who were pronounced dead outside the city limits of Alliance, Canton, or Massillon.
Please note, that under Ohio law, anyone can obtain a death certificate for a death that was pronounced in our jurisdiction WITHOUT the decedent’s social security number.
If you need a certified copy of a death certificate WITH the decedent’s social security number, you must be one of the following:
- The deceased’s spouse, or lineal descendant
- The deceased’s executor, attorney, or legal agent
- A representative of an investigative government agency
- A private investigator
- A funeral director (or agent responsible for disposition of the body) acting on behalf of the deceased’s family
- A veteran’s service officer
- An accredited member of the media
You will have to bring proof or attach a copy of proof of one of the above reasons.
There are several ways you can obtain a birth or death certificate:
On line: For your convenience, you can process online requests through an independent company that the Stark County Health Department has partnered with to provide you this service; VitalChek Network, Inc. VitalChek can be reached through their website. An additional fee is charged by VitalChek for this service, and all major credit cards are accepted, including American Express, Discover, MasterCard or Visa. Credit cards may be used by clicking HERE.
Through the mail: You may send a check for $25.00 to the “Stark County Health Department”. Please also include the name and date of birth of the person whose certificate you are ordering. Also include your name and a return mailing address.
